Should Client not provide Valet Health with the New Patients Monthly Export each month, ROI data calculations will be based on the Valet Health database national average for lead-to-new-patient conversion rate by specialty.
• Payment Terms & Method: Credit card on file required prior to work start. This is a 6-Month program agreement for LaunchPad. Notice of Cancellation must be provided at least 30 days prior to the end of the contract term by email. At the end of the contract term, contract will automatically renew.

• LaunchPad Provider Data Management for WebMD, Vitals, Google Business Profiles, etc for physician(s) actively onboarded to LaunchPad with monthly dashboard reporting insights
• QR code directs to physician review capture link for patient online reviews acquisition across high profile sites, e.g. Google, WebMD, Vitals, etc.
NOTE: US News pulls data from Doximity.com -- Doximity.com is social networking for physicians and Valet Health recommends that physician specifically claims and update Doximity which will also update US News. In addition, due to recent policy changes at Healthgrades, Healthgrades.com does not allow any 3rd party software systems like LaunchPad to access or managed profiles, so it is incumbent upon the provider to claim and update his or her Healthgrades.com profile.
• Appointment Request Landing Page: This scope also includes the development of a Provider Appointment Request Landing Page linked from a physician-focused Google Business Profile with new patient referral leads captured and measured.
